This dialog box is used to store information on the bridge as deemed appropriate by the engineer. Perform the following steps to store information on the bridge:

1. Enter the Bridge Number (alphanumeric).
2. Enter the Bridge Code (ID) (alphanumeric)
3. Enter the Job Number (alphanumeric).
4. Enter the date.
5. Enter the Engineer's name or initials
6. Enter any appropriate textual information
7. Enter the type of inspection (e.g. Bi-annual).
8. Enter the Condition Rating.
9. Enter the Sufficiency Rating.
